Hardware error Tracking servo faillure

With my Imation DVD+R 16x

I burn with 4x or 8x and I have same error.
Sense code = 0x09, 0x01
Tracking servo faillure

First thing I would try is a different brand of media, like Verbatim. If you still have the problem, it could be the drive is going bad.
