I have a Power mac G5, OS 10.3.9
Drive: Sony DVD RW DW-U21A
I have tried to apply the SuperDrive Updater 1.0 from Apple, it was not needed.
I use Toast Titanium 7.1.2
PROCEDURE:
I insert a blank DVD-R (Sony16D1 or Verbatim MCC03RG20)
Write speed (I tried 1X, 2X, 4X, 1x DVD, 2x DVD )
RESULT: Always the same result:
Preparing...
Writing Lead-Out...
Then I IMMEDIATELY get error: The disk failed to be written [OK]
I press [OK] the disk ejects.
Can anyone help!?

I'm confused by the write speeds that you had available to select. Were those the speeds available after the blank disc was inserted and ready?
I don't know what may be the cause of this. Toast goes straight from Preparing to Writing lead out? Very strange. Try choosing Save as Disc Image from the File menu and then burn the disc image using the Image File setting in the Copy window. Does this work?

If I select first, I get all those options. When the disc is inserted, only DVD speeds are available.

Try this: Select the .toast disc image file you saved in the Finder and change its extension from .toast to .iso. Now open Disk Utility. Drag the disc image to the left frame in the Disk Utility window. Select it in the window and click the Burn button. What I want to find out is if your DVD drive will burn the DVD while Toast is not doing this.

Here is the result after following your instructions:
I selected 1x to be sure in Disk Utility.
press <BURN>
Progress: Burning Image MyDisk.iso
Writing Track; but the progress does not move
2 minutes later the tray open
Progress: STOP The device failed to respond properly, unable to recover or retry
I got a burner problem? How can I check?!

The first thing I'd try is a laser-cleaning disc to learn if the problem may just be some dust on the laser.
