please help im Getting different track names after burning on toast and playing it in itunes or wmp

I burned a cd using toast 6 platinum then play it in windows media player (pc) and itunes(mac) when it plays it created a different track names, what causes this? and how will i avoid it?
correct naming of the track and album is very important for me cause this cd goes to radio station
can someone please help me
