How To Claim Rebate On Downloaded Roxio Creator 2011

Okay, I always thought I was pretty good at figuring things out. I paid for and downloaded Roxio Creator 2011. Part of the order said "Rebate ($20 Mail-In). Darned if I can find a file that has a form or something for me to fill out and mail in to get my rebate! I must be missing something... what am I missing here?

Perhaps this will help or get in touch with Roxio Customer Service. We are just users of the software and have no information on the rebate system
