I've had this card for about 2 months now, and while it's been glitchy, I've never seen this issue before.
My setup is this:
XBOX 360 is connected to the capture card via HDMI (720p), capture card HDMI out to the television. I can see the image on the television screen, but as soon as I open up Roxio Game Capture HD Pro, and select "Capture", my laptop screen goes black, and I can see about a inch of the top of the screen shaking back and forth.
My laptop is equipped properly. Like I said, I've had this card now for 2 months, and while it's had its problems, nothing like this. I've restarted twice, haven't installed or updated anything since the last time I used it, and have tried different USB slots. I also tried all options through the XBOX, (720, 1080i, 1080p).
